Description Benefits Supplemental Questions The City of Norfolk's Community Services Board Department is seeking an experienced Licensed Practical Nurse (LPN) for our Integrated Care Clinic. The LPN will provide psychiatric nursing support through the coordination of medical services and administration of medication to clients with co-occurring MI/SA/ID within the Integrated Care Clinic. Essential Functions: Performs psychiatric nursing services such as medication administration, and delivery, evaluation patients using the AIMS (Abnormal Involuntary Movement Scale-Revised) rating scale, and obtaining vital signs including height, weight, and waist circumferences and BMI. Screens and manages staff and patient phone calls, emails and other patient related concerns to determining the necessary care to provide while providing excellent customer service . Administers medication, immunizations and treatments by reviewing and clarifying the doctor's orders, and recording medication administration accurately. Process Patient Assistance Program applications, coordinates transfer of PAP medications to pharmacist for dispensing; delivers PAP medications to patients. Provides care coordination for approximately 160 patients. Maintains medication logs, package medications received from the authorized contract pharmacy, and process medications for return to the authorized contract pharmacy. Destroy outdated and discontinued medications according to established procedures. Education/Experience: Work requires knowledge of a specific vocational, administrative, or technical nature which may be obtained with a two year associate's degree, diploma or equivalent from a college, technical, business, vocational, or correspondence school. Appropriate certifications may be awarded upon satisfactory completion of advanced study or training. One year of experience in a medical environment performing patient care duties . The ideal Candidate will possess the following qualifications: A current Virginia LPN License Two (2) years experience working as a LPN with at least one year psychiatric nursing experience Two (2) years experience working in a fast paced medical setting Additional Information & Requirements: Must be certified or certifiable in Basic First Aid and CPR.
